Medical and Vision Insurance Policy

 Vision Plans (Open Access)

Outreach Vision is an “Open Access” provider.  This will allow you to receive all of your plan’s vision benefits and avoid many of your vision plan’s limitations.   We can evaluate your vision plan’s open access benefits option through a database called Anagram.  We will file a courtesy claim with your vision plan provider through Anagram and apply your vision plan’s benefits to your total charges.    

Medical Insurance Plans (Out of Network)

Outreach Vision charges are always the patient’s responsibility.  When you have a medical vision diagnosis, Outreach Vision will be happy to assist you with reimbursement from your insurance provider.   Outreach Vision can file a courtesy claim of your charges when you provide the insurance company’s contact information.  All insurance reimbursement arrangements are between you and your insurance provider. 

Medicare and Medicaid

Outreach Vision accepts assignment for Medicare and Medicaid patients.  As a courtesy, we will also bill your secondary insurance provider directly.  You will be asked to sign an Advanced Beneficiary Notice for the services not covered under Medicare.  You will be responsible for covering these services. 

Membership Plans

Outreach Vision also offers “in-house” Ambassador Membership Plans for patients who do not have access to employee offered benefit plans.  This allows patients to receive annual examination benefits at a reduced cost and discounts on eyewear, contact lenses and sunglasses.
