At Outreach Vision, we offer patient forms online so you can complete them in the convenience of your own home or office. Fax or email us your completed forms two days prior to your appointment.​​​​​​​

• Download the necessary form(s) and fill in the required information.

  • Adults: Patient Registration Form and BVDQ (For Ages 14 & Older)

  • Child: Patient Registration Form, Extended History Form-Child (for school aged children), & age appropriate BVDQ.

• Please fill out and submit your Patient Registration two days prior to your appointment.
